Well Doctor Visits

Be Prepared: Before your yearly check-up it would be wise to take the time to do the following. We suggest this because most healthcare professionals have a limited time to spend with each patient.

1) Go to your DISEASE risk TOOL . This will help you decrease your odds of getting certain cancers and other chronic diseases by asking you questions about your health, lifestyle and personal background.

2) Go to KNOW YOUR ROOTS to print and fill in Your Family Medical History Tree (PDF). This will help you determine which cancers and other chronic diseases you may be more at risk for according to your personal family medical history.

3) Print out YOUR PERSONAL CHECKLIST FOR HIM (this is a PDF) or YOUR PERSONAL CHECKLIST FOR HER (this is a PDF).

Ask Questions: Don’t be afraid to ask the questions we have listed on Your Check List,especially the ones noted in boldface, to review with your doctor. A yearly check-up is your best opportunity to stay healthy by getting yourself checked as comprehensively as possible by your doctor or healthcare professional.

Get Answers: Make sure you understand your doctor’s answers. Ask questions until you do. Take notes.
